Inclusion criteria were as follows: a ratio of arterial oxygen tension to fractional inspired oxygen of less than 300 mmHg at zero end-expiratory pressure; bilateral hyper-densities on a bedside chest radiogram; and pulmonary capillary wedge pressure below 18 mmHg and/or left ventricular ejection fraction greater than 50%, as estimated by transoesophageal echocardiography.
There were differences in study design and methods (though most were quite similar), fenfluramine or dexfenfluramine doses (though in most studies, doses were similar), additional drugs used, patient characteristics, interval between cessation of therapy and echocardiography, and completeness of follow-up.
The Center constructs a file of medical records in a standardized, subdivided sequence (hospital admission notes and discharge summaries; neurologic consultation notes; reports of computed tomographic and magnetic resonance imaging of the head; reports of imaging of the heart by transthoracic and transesophageal echocardiography; copies of electrocardiograms; reports of imaging of cervicocephalic vasculature by angiography using conventional, computed tomographic, or magnetic resonance techniques or by ultrasonography; and reports of blood work).
The probe was connected to an ultrasonograph device (Color Doppler, model SSD-830; Aloka Company, Tokyo, Japan) and videorecording of two-dimensional transoesophageal echocardiography images were made for later analysis.
Initial reports of remarkably high prevalence of aortic and mitral regurgitation (AR and MR) as assessed by echocardiography in five separate populations [ 2 ] were subsequently supported by most [ 3 4 5 ] but not all investigators [ 6 ] . These uncontrolled reports were soon followed by a series of studies in which prevalence of echocardiographically-defined AR and MR were assessed in persons who had taken fenfluramine, dexfenfluramine, phentermine, or combinations of these medications and compared to unexposed, matched control populations [ 7 8 9 10 11 12 13 14 15 ] . The most recent of these studies found 30% of persons currently taking fenfluramine or dexfenfluramine had mild or greater AR or moderate or greater MR, similar to the very high prevalence reported in older studies [ 15 ] . The only prospective study found a significant, exposure-related increase in the risk of clinically diagnosed valve disease among users of fenfluramine and dexfenfluramine, but not phentermine [ 16 ] .
